• Andersonville Internal Medicine charges an annual fee of $5000, with a 10% discount for spouses/partners.  Included in the membership is a comprehensive physical exam and consultation with your internist (see below). There are no additional charges for office visits, phone calls, virtual care, or any communication with your doctor, and there are no co-pays. 

    Payment is charged upon sign-up and then annually.

    There is no fee for payment with ACH direct debit or check. Members will pay a 2.75% fee for credit card processing.  We do not accept American Express.

    • Men’s health care including testicular health, erectile dysfunction, benign prostatic hypertrophy (BPH/enlarged prostate), prostate cancer screening, sexual health, and low testosterone (hypogonadism).

    • Comprehensive inclusive, and respectful LGBTQI+ care including PrEP, HIV management, STI screening and treatment, and sexual and mental health support. 

    • In-office testing with immediately available results including urine dipstick, pregnancy testing, strep, covid and flu testing

    • Vaccine administration* including Tdap (tetanus-diphtheria-pertussis), shingles (Shingrix), pneumonia (Prevnar-20), respiratory syncytial virus, hepatitis A, hepatitis B, influenza, and COVID.

    • Lab testing in our office for ongoing care or new evaluations*

    *Additional lab testing outside the annual physical labs and all vaccines will be billed to your insurance by Quest Labs and VaxCare, respectively

    • 2–3-hour initial consultation and physical including expanded lab panel, EKG, and vision screening.

    • Comprehensive review of medical and family history.

    • Evaluation of current medical concerns.

    • Review preventative care and screening recommendations in detail. 

    • Thorough evaluation of lifestyle factors affecting health including sleep, nutrition, exercise, stress management, and relationships. 

    • Physician and patient will collaborate to create a plan for ongoing care and health goals.

    • Referrals to specialists provided on annual visits with personal recommendations taking advantage of our robust specialist network across 

    • Chicagoland and spanning multiple academic medical institutions.

  • We will see you the same day or the next day (Monday-Friday) for any acute concerns. In most cases we can see new patients for an initial intake within a few weeks.

  • No, our medical services are covered by your annual fee. In some cases we may be able to provide you a “Superbill” to submit to your insurance for reimbursement as an out of network provider.  We cannot do so for Medicare.

  • Yes, your insurance will be billed for any lab work outside of the annual physical labs and any vaccines done in our office and for any testing, labs, or imaging done outside of our office. In addition, you will need your insurance for specialist visits, emergency room visits, and hospitalizations.

  • Patients may use their HSA account for partial reimbursement of the membership fee, within the allowable tax structure.

  • During office hours, you may call the office, text us using our secure app, or message us on the Healow app, which imports directly into your medical chart. After hours, you may reach your doctor directly for emergency care.

  • Our electronic medical record, EClinical Works, allows us to see your records from any provider who uses the EPIC EMR system (this includes most major medical centers in the US). We are also able to request your records electronically if needed from any provider. As the quarterback of your health care team, we ensure your other providers have our thorough records as well. You will also be able to access our notes and records on our patient portal app, Healow. 

  • You can get lab work done at our office, or we can provide an order for you to get labs done at a facility close to your home or work. You may get diagnostic testing like X-rays, scans, or heart testing done at a facility of your choice. We are able to order directly into the Northwestern, University of Chicago, and Rush systems, or we can provide an order for you to take to your preferred facility.

  • Our goal is to keep you out of the emergency room and urgent care facilities! However, if an emergency occurs, your doctor will be available to speak with the treating physician in the ER and the hospitalist team to ensure they have all the information they need about you and to consult on your care. Your doctor will then see you after your ER visit or hospitalization for follow up. 

  • We are not affiliated with a particular hospital system. However, we do have the ability to order labs and diagnostic testing directly into the Northwestern, Rush, and University of Chicago systems. All of our physicians have longstanding, deep ties to many specialists at major facilities across the Chicagoland area, as well as a network of private practice specialists with good availability. We will work with you to find the right specialist and the right facility for your needs.
